New Patient Form Digitilization
We're excited to announce that new patients now have the option to fill in their intake forms online. All content of the patient forms remain the same, however the layout is slightly different due to digital constraints. With this new method, patients and caretakers can easily fill out their intake forms at their earliest and most convenient time. Patients' digital privacy is our greatest concern - once the form is submitted, the patient's information is encrypted and sent to our office's database.
In choosing to use this new option, patients must fill out all 6 forms in order to successfully complete their intake form. To do so, patients must separately submit each one. Each form will not autosave - we recommend patients to fill out and submit one form in one sitting. There is no time constraint to filling out these forms successively - after the form submission, the individual form will be sent to our office and filed under the patient name. For smooth operations and convenience for both patient and staff, please remember to fill out all 6 forms before the next scheduled visit if you choose to proceed with this option.
The 6 forms are listed under this tab (Patient Forms) as their own separate sub-tab. The 2 tabs of Barthel Index Activity and Functional Activities Questionnaire (labelled as "FA form") are meant to be filled out by the patient's caregiver. The other 4 tabs of General Patient Information, Family History, Medications, and Symptoms forms are meant to be filled out by the patient themselves. These forms are not locked after a user fills and submits it - we recommend patients to write down a list for all completed forms to avoid duplicate form submissions and confusion.
